Position Overview:
We are seeking a qualified Registered Nurse (RN) to fill the dual role of Homecare Case Manager and possible Home Care Aid Trainer. The ideal candidate will possess strong clinical skills, exceptional communication abilities, and a passion for educating and empowering caregivers. This position offers a unique opportunity to make a meaningful impact on the lives of our clients and contribute to the professional development of our caregiving team.
Responsibilities:
- Conduct comprehensive assessments of clients’ medical needs, develop individualized care plans, and monitor their implementation.
- Serve as the primary point of contact for clients, families, and caregivers, coordinating all aspects of care delivery and ensuring continuity of services.
- Provide hands-on training and supervision to home care nurses and aides, including instruction on personal care tasks, medication administration, and safety protocols.
- Conduct in-home visits to assess caregiver competency, provide ongoing support and education, and ensure adherence to care plans.
- Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to optimize client outcomes and promote holistic care.
- Maintain accurate and thorough documentation of client assessments, care plans, and interventions in compliance with regulatory standards and organizational policies.
Qualifications:
- Current licensure as a Registered Nurse (RN) in the state of Washington.
- Minimum of one year preferred, of clinical experience, with a preference for candidates with prior experience in homecare or case management.
- Strong assessment and critical thinking skills, with the ability to develop and implement individualized care plans.
- Excellent communication and interpersonal abilities, with a compassionate and client-centered approach to care.
- Experience in training and mentoring caregivers preferred, with a commitment to promoting excellence in caregiving practices.
- Proficiency in computer skills, including electronic health record documentation and Microsoft Office applications.
Additional Qualifications:
- Willingness to travel throughout Pierce County to conduct in-home visits and caregiver training sessions.
- Strong commitment to continuous learning and professional development, with a willingness to acquire new skills and adapt to evolving care practices.
- Exceptional customer service skills, with the ability to establish rapport, communicate effectively, and advocate for clients’ needs.
- Experience in case management, including proficiency in conducting assessments, developing care plans, and coordinating services across multiple providers.
- Prior experience in home health or community-based care settings preferred, with a demonstrated understanding of the unique challenges and opportunities in delivering care in the home environment.
